What is Teen LifeCheck?

Teen LifeCheck is a new website from the NHS for people aged between 12 and 15. It’s an online quiz that asks you all sorts of questions about your life. There are questions about health, keeping fit, staying safe, alcohol, drugs and loads more. There are no right or wrong answers, and all the answers given are completely private.

Once you’ve answered all the questions, the website will let you know how you’re getting on with life and give you lots of tips and advice. You’ve probably got all kinds of questions going around your head, maybe about friends, school, family, smoking, drugs, alcohol and sex. Sometimes you just need a private place to go where you can see how you’re doing and get some advice.

Teens will be able to try NHS Teen LifeCheck at Lewisham shopping in North Square and will then be able to compete to win a Nintendo Wii simply by being the highest scorers playing Dance Revolution: Hottest party 2 or Mario & Sonic at the Olympic Games.

Competition terms and conditions:

Competition starts on the first day at each location and closes on the last day at each location. The parents or guardians of the winners will be notified by telephone by 04.09.09

How to enter:
1. Entrants must have completed the NHS Teen LifeCheck online questionnaire, located on the NHS Teen LifeCheck stand in order to be eligible to take part in the Win a Wii competition. 2. Entrants who have completed the NHS Teen LifeCheck online questionnaire will then be permitted to play on one of the two Wii’s located on the NHS Teen LifeCheck stand. There will be a choice of either Dance Dance Revolution: Hottest Party 2 or Mario & Sonic at the Olympic Games (Athletics - 100m) to play. 3. Entrants are only permitted to have one attempt at either of the available games.4.The winners will be the top scorers of Dance Dance Revolution: Hottest Party 2 or Mario & Sonic at the Olympic Games (Athletics - 100m) respectively. 5. The winner will be the top scorer over the two days per location.

Terms and conditions:
Winner’s names will be available on request. If you would like to request this information please send a self addressed envelope to: NHS Teen LifeCheck Win a Wii Competition, lime, Warwick Building, Kensington Village, Avonmore Road, London, W14 8HQ.There are a total of 36 Wii’s to win. Only two Wii’s to be won per location. There is no cash alternative. Only one entry per person per location allowed. Your name, address and parents/guardian telephone number will be collected when you complete your game on the Wii. Your data will not be kept on record or distributed to any third parties and is only recorded for distribution of prizes. In the case of two identical top scores the winner will be drawn by an independent judge.

All prizes will be sent out within 28 days of notification of winning. Winners may be required to take part in post activity promotion. The NHS LifeCheck competition is only open to young people aged between 12 and 15 years old. No proof of purchase required. There is no cost to enter the competition.
